Acknowledgment and Assumption of Risk
I understand that participation in water polo involves physical contact, water hazards, and other risks which may result in serious injury or death. I voluntarily assume all such risks associated with this tournament.
Release of Liability
I release and hold harmless Motor City Water Polo, its organizers, coaches, and affiliates, and Birmingham Public Schools, from any claims or damages resulting from my participation, even if due to negligence.
Indemnification
I agree to indemnify and hold harmless the Released Parties from any legal claims arising out of my (or my child’s) participation.
Medical Authorization
I authorize emergency medical treatment and agree to be financially responsible for any resulting care.
Governing Law
This waiver is governed by Michigan law. Disputes shall be resolved exclusively in Oakland County.
Severability
If part of this waiver is invalid, the rest remains in effect.
